At OHR Health and Wellness in Elgin, Illinois, our board-certified psychiatric mental health nurse practitioner Jaya Luis, APRN, PMHNP-BC, compassionately cares for patients 4 and older via telehealth. She treats patients like family, finding the most appropriate diagnoses and treatment plans to help them thrive.

OHR Health and Wellness cares for patients with attention-deficit/hyperactivity disorder (ADHD), bipolar disorder, depression, generalized anxiety disorder (GAD), post-traumatic stress disorder (PTSD), schizophrenia, and other behavioral and mental health concerns. Jaya also helps those with insomnia sleep soundly and wake up energized.

OHR Health and Wellness provides outpatient psychiatric evaluation and medication management across the lifespan through telehealth services. The practice also offers onsite psychiatric care for residents of assisted living and memory care communities. Services are patient-centered, evidence-based, and billed directly to patient insurance or private pay.

What to Expect
at Your First Visit

Your initial appointment includes a comprehensive psychiatric evaluation. We will review your history, current concerns, and treatment goals together. Please allow adequate time to complete intake forms prior to your visit.

Our Medication Philosophy Medication decisions are made collaboratively. When medications are recommended, we aim to use the lowest effective dose and regularly reassess benefits and side effects.

Telehealth Services

All outpatient services are provided via HIPAA-compliant telehealth platforms. Patients must be physically located in Illinois at the time of service. Telehealth services are not appropriate for all conditions and may be adjusted based on clinical judgment.

In-Person Care

In-person psychiatric services are provided for residents of Assisted Living and Memory Care communities through coordinated on-site visits.
